href=\"https:\/\/kffhealthnews.org\/morning-breakout\/senate-blocks-rival-health-care-bills-aca-premiums-expected-to-soar\/\" target=\"_blank\" rel=\"noreferrer noopener\">votes<\/a>&nbsp;late last year and no&nbsp;subsequent&nbsp;bipartisan agreement, the enhanced premium tax credits expired as of January 1.&nbsp;Some states, particularly those operating State-Based Marketplaces (SBMs), have been&nbsp;preparing for&nbsp;this possibility for months and are moving to blunt the impact on consumers&nbsp;by implementing their own state-funded subsidies&nbsp;and implementing other programs&nbsp;aimed at&nbsp;stabilizing&nbsp;the cost of unsubsidized premiums.&nbsp;<strong><\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-state-specific-subsidies\">State-Specific Subsidies<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">SBMs have the flexibility under the Affordable Care Act to offer&nbsp;additional&nbsp;state-based subsidies on top of federal premium tax credits to further lower&nbsp;monthly&nbsp;premium payments for&nbsp;Marketplace enrollees.&nbsp;A few SBMs have enacted their own supplemental premium subsidies to&nbsp;maintain&nbsp;affordability&nbsp;and enrollment&nbsp;now that the&nbsp;enhanced premium tax credits&nbsp;have&nbsp;lapsed.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>New Mexico has&nbsp;advanced two measures that would backfill the lost enhanced premium tax credits&nbsp;in their entirety for all consumers&nbsp;in 2026.&nbsp;<\/strong>BeWell, New Mexico\u2019s Health Insurance&nbsp;Marketplace,&nbsp;<a href=\"https:\/\/api.realfile.rtsclients.com\/PublicFiles\/6c91aefc960e463485b3474662fd7fd2\/15a6c1dd-e12b-4ffb-95af-bb54262218f3\/FINAL-PY26%20%20MAP%20Policy%20and%20Procedures%20Manual.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">will backfill<\/a>&nbsp;all of&nbsp;the lost federal tax credits for enrollees with annual incomes up to 400% FPL.&nbsp;Additionally, for enrollees making above 400% of poverty, New Mexico financial&nbsp;assistance&nbsp;<a href=\"https:\/\/api.realfile.rtsclients.com\/PublicFiles\/6c91aefc960e463485b3474662fd7fd2\/50c9a4d0-d5c8-48aa-8b9a-5c43e8fa23bc\/Addendum%201_MAP%20P%26P%20Middle%20Income%20Household.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">will cap<\/a>&nbsp;premium payments for a benchmark plan at 8.5% of their household income,&nbsp;mirroring the structure of&nbsp;the enhanced premium tax credits.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Other states&nbsp;have moved to&nbsp;fully&nbsp;backfill the expired tax credits&nbsp;for&nbsp;a portion&nbsp;of&nbsp;enrollees.&nbsp;<\/strong><a href=\"https:\/\/insurance.maryland.gov\/Documents\/newscenter\/newsreleases\/2026-ACA-Press-Release-Approved-Rates-with-exhibits.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Maryland<\/a>, for example, adopted a&nbsp;single-year&nbsp;state&nbsp;premium&nbsp;assistance program that replaces 100% of the lost federal subsidy for enrollees below 200% of the federal poverty level (FPL)&nbsp;($31,300 for an individual signing up for coverage in 2026) and&nbsp;partial replacement&nbsp;of the lost enhanced tax credit for those with incomes&nbsp;above&nbsp;200%&nbsp;up to&nbsp;400% FPL.&nbsp;However, there&nbsp;is no&nbsp;additional&nbsp;state&nbsp;assistance&nbsp;to&nbsp;replace&nbsp;tax credits lost&nbsp;by&nbsp;people with annual incomes above 400% FPL, who are&nbsp;now completely ineligible for any tax credits with the reinstatement of the \u201c<a href=\"https:\/\/www.kff.org\/affordable-care-act\/mapping-the-uneven-burden-of-rising-aca-marketplace-premium-payments-due-to-enhanced-tax-credit-expiration\/\" target=\"_blank\" rel=\"noreferrer noopener\">subsidy cliff<\/a>.\u201d&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.coveredca.com\/newsroom\/news-releases\/2025\/08\/14\/covered-california-rates-and-plans-for-2026-consumer-affordability-on-the-line-with-uncertainty-surrounding-federal-premium-tax-credit-extension\/#:~:text=This%20year%2C%20California,federal%20poverty%20level.\" target=\"_blank\" rel=\"noreferrer noopener\">California<\/a>&nbsp;is&nbsp;allocating&nbsp;funds&nbsp;to fully replace the premium tax credits for enrollees making up to 150% FPL and partially&nbsp;replace&nbsp;the lost credits for those with incomes between 150% and 165% FPL&nbsp;in 2026.&nbsp;With California&nbsp;<a href=\"https:\/\/hbex.coveredca.com\/data-research\/library\/Brief%201%20IRA%20ACA%20Premium%20Impacts%202025.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">receiving<\/a>&nbsp;about&nbsp;$2 billion&nbsp;annually in enhanced premium tax credits,&nbsp;only a small share&nbsp;of the federal tax credits will be backfilled by these state-specific subsidies.&nbsp;Like in Maryland, California also does not have any&nbsp;additional&nbsp;state&nbsp;assistance&nbsp;to replace&nbsp;enhanced premium tax credits&nbsp;for people with annual incomes above 400% who have lost eligibility&nbsp;for&nbsp;any&nbsp;federal&nbsp;premium&nbsp;assistance&nbsp;since January 1.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Some&nbsp;states, like&nbsp;Colorado&nbsp;and&nbsp;Washington,&nbsp;have&nbsp;also&nbsp;retooled or created state-specific subsidy programs to&nbsp;provide an&nbsp;additional&nbsp;flat dollar amount&nbsp;to enrollees.&nbsp;<\/strong>However, in many cases, these dollar amounts fall short of entirely backfilling lost federal tax credits<strong>.<\/strong>&nbsp;In&nbsp;<a href=\"https:\/\/connectforhealthco.com\/financial-help\/colorado-premium-assistance\/\">Colorado<\/a>,&nbsp;a&nbsp;maximum&nbsp;dollar amount&nbsp;of $80&nbsp;per month&nbsp;for an individual enrollee (and an&nbsp;additional&nbsp;$29 for each&nbsp;subsequent&nbsp;family member)&nbsp;is&nbsp;provided&nbsp;to&nbsp;households&nbsp;making between 100% to 400% of poverty.&nbsp;This&nbsp;plan&nbsp;will backfill about 40% of the lost federal&nbsp;assistance.&nbsp;Washington is retooling their existing&nbsp;<a href=\"https:\/\/www.wahbexchange.org\/about-the-exchange\/initiatives\/cascade-care\/cascade-care-savings\/\" target=\"_blank\" rel=\"noreferrer noopener\">Cascade Care Savings<\/a>&nbsp;program, setting new fixed dollar maximums for 2026&nbsp;to provide some relief to consumers&nbsp;($55 per member per month for those receiving federal tax credits and $250 per member per month for those not receiving subsidies).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Prior to the creation and expiration of the enhanced tax credits, some SBMs already had additional state assistance in place on top of federal premium tax credits<\/strong>.&nbsp;<a href=\"https:\/\/www.kff.org\/faqs\/faqs-health-insurance-marketplace-and-the-aca\/help-paying-marketplace-premiums-the-basics\/which-states-offer-additional-financial-assistance-for-marketplace-plans\/\" target=\"_blank\" rel=\"noreferrer noopener\">States<\/a>&nbsp;such as&nbsp;<a href=\"https:\/\/info.nystateofhealth.ny.gov\/costsavings\" target=\"_blank\" rel=\"noreferrer noopener\">New York<\/a>,&nbsp;<a href=\"https:\/\/www.accesshealthct.com\/financial-help\/\" target=\"_blank\" rel=\"noreferrer noopener\">Connecticut<\/a>,&nbsp;<a href=\"https:\/\/info.healthconnect.vermont.gov\/financial-help#:~:text=Vermont%20Premium%20Assistance,cost%20Silver%20plan).\" target=\"_blank\" rel=\"noreferrer noopener\">Vermont<\/a>,&nbsp;<a href=\"https:\/\/www.mahealthconnector.org\/help-center\/resource-download-center\/2026-consumer-guide-to-subsidies\" target=\"_blank\" rel=\"noreferrer noopener\">Massachusetts<\/a>, and&nbsp;<a href=\"https:\/\/www.nj.gov\/getcoverednj\/financialhelp\/premiums\/\" target=\"_blank\" rel=\"noreferrer noopener\">New Jersey<\/a>&nbsp;all have state-specific subsidies that are not&nbsp;directly related&nbsp;to the&nbsp;expiration&nbsp;of the enhanced premium tax credits and will remain in place&nbsp;whether or not&nbsp;they are extended.&nbsp;Additionally,&nbsp;New York and Oregon&nbsp;operate&nbsp;a&nbsp;<a href=\"https:\/\/www.medicaid.gov\/basic-health-program\" target=\"_blank\" rel=\"noreferrer noopener\">basic health plan<\/a>&nbsp;or similar program&nbsp;that provides coverage to certain low-income residents who would otherwise be eligible for Marketplace plans, offering lower premiums and cost sharing regardless of changes to federal&nbsp;tax credit&nbsp;policy.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-reinsurance-programs\">Reinsurance Programs<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Several states also&nbsp;operate&nbsp;<a href=\"https:\/\/www.cms.gov\/marketplace\/states\/section-1332-state-innovation-waivers\" target=\"_blank\" rel=\"noreferrer noopener\">Section 1332<\/a>&nbsp;reinsurance programs that reduce&nbsp;unsubsidized&nbsp;premiums by reimbursing insurers for a share of high-cost claims. These programs do not replace lost federal subsidies, but they help stabilize the&nbsp;unsubsidized&nbsp;premiums&nbsp;some&nbsp;consumers will face&nbsp;the full cost of&nbsp;in 2026.&nbsp;Specifically,&nbsp;these are&nbsp;individuals and families&nbsp;with annual incomes over 400% FPL who will be entirely locked out of eligibility&nbsp;for financial&nbsp;assistance&nbsp;with the reinstatement of the \u201c<a href=\"https:\/\/www.kff.org\/affordable-care-act\/mapping-the-uneven-burden-of-rising-aca-marketplace-premium-payments-due-to-enhanced-tax-credit-expiration\/\" target=\"_blank\" rel=\"noreferrer noopener\">subsidy cliff<\/a>\u201d&nbsp;and face&nbsp;some of the largest increases&nbsp;in premium payments.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/insurance.maryland.gov\/Documents\/newscenter\/newsreleases\/2026-ACA-Press-Release-Approved-Rates-with-exhibits.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Maryland\u2019s reinsurance program<\/a>, in place since 2019 and extended through 2028, has lowered premiums by&nbsp;as much as&nbsp;35%&nbsp;relative&nbsp;to what they would have been, according to&nbsp;state&nbsp;findings,&nbsp;and&nbsp;is&nbsp;expected to continue softening premium growth when enhanced premium tax credits expire.&nbsp;<a href=\"https:\/\/doi.colorado.gov\/sites\/doi\/files\/documents\/EPTC%20Impact%20Slides.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Colorado<\/a>,&nbsp;<a href=\"https:\/\/www.nj.gov\/dobi\/division_insurance\/section1332\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\">New Jersey<\/a>,&nbsp;<a href=\"https:\/\/oci.georgia.gov\/document\/document\/1332-waiver-public-forum-2025-07-01\/download\">Georgia<\/a>,&nbsp;and&nbsp;<a href=\"https:\/\/dfr.oregon.gov\/business\/reg\/health\/Documents\/reinsurance-program\/innovation-waiver-2023-2027-app.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Oregon<\/a>&nbsp;operate&nbsp;similar programs.&nbsp;Colorado and New Jersey&nbsp;say that these programs have&nbsp;reduced&nbsp;statewide&nbsp;unsubsidized&nbsp;premiums by&nbsp;roughly 20%&nbsp;and provide&nbsp;even greater relief in rural rating areas.&nbsp;In&nbsp;<a href=\"https:\/\/www.cms.gov\/cciio\/programs-and-initiatives\/state-innovation-waivers\/downloads\/1332-ga-waiver-acumen-analysis.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Georgia<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/dfr.oregon.gov\/news\/news2025\/pages\/20251008-final-health-rates-2026.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Oregon<\/a>,&nbsp;their reinsurance programs have reduced the cost of unsubsidized premiums by at least 10%.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-enrollment-assistance\">Enrollment Assistance<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">On top of&nbsp;state-based subsidies and reinsurance programs, some states may also increase outreach to help consumers shop for lower-cost options. But navigator programs have faced repeated&nbsp;<a href=\"https:\/\/www.kff.org\/quick-take\/a-90-cut-to-the-aca-navigator-program\/\" target=\"_blank\" rel=\"noreferrer noopener\">federal funding cuts<\/a>, limiting the ability of states,&nbsp;particularly those without robust state-based outreach budgets,&nbsp;to rely on enrollment&nbsp;assistance&nbsp;as a tool for mitigating affordability challenges.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-limited-relief\">Limited Relief<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A small subset of states&nbsp;(and just a&nbsp;portion&nbsp;of those that&nbsp;operate SBMs)&nbsp;have moved to&nbsp;blunt the impact of&nbsp;the&nbsp;enhanced premium tax credits&nbsp;expiration&nbsp;on&nbsp;enrollees.&nbsp;While expanded state subsidies and reinsurance programs may soften the impact of the enhanced premium tax credit&nbsp;expiration&nbsp;for some consumers, they are unlikely to&nbsp;substantially alter&nbsp;the projected coverage losses.
